疾病定義與分類
- 脂肪肉瘤（liposarcoma）雖為最常見的軟組織肉瘤之一，但少見原發於皮下，僅例外原發於真皮。
- 傳統分為三亞型：well-differentiated / dedifferentiated；myxoid（含 cellular／舊稱 round cell 變異型）；pleomorphic。
各亞型臨床特徵
- Dedifferentiated：去分化指雙相腫瘤含 atypical lipomatous tumor 與分級不一的肉瘤性未分化成分。主要發生於深部腫瘤（尤其腹膜後），皮下病灶屬例外。
- Myxoid：發生於成人，發病高峰於第四至第五十年，無性別偏好。兒童青少年罕見，但於該年齡層為最常見脂肪肉瘤型。多數源於深部軟組織，皮下腫瘤極罕見；最常見部位為下肢（尤其大腿）。所有腫瘤無例外傾向局部復發，約 1/3 病例發生轉移（視分級而定）。cellular 成分、壞死與 p53 過度表現與不良預後相關。round cell liposarcoma 一詞於 WHO 分類已不再採用。
- Pleomorphic：最少見的 liposarcoma 變異型，無性別偏好，病人為老年，腫瘤深在、主要見於四肢。僅例外發生於真皮或皮下。腫瘤生長快速、高度傾向局部復發與轉移。雖於深部軟組織為最少見的 liposarcoma，卻可能是最常以皮膚原發出現者。
致病機轉
- Dedifferentiated：與 atypical lipomatous tumor（well-differentiated liposarcoma）共有相同細胞遺傳變化，即環狀或巨型標記染色體伴 12q13~15 區（含 MDM2）擴增與過度表現；另可見 JUN（1p32）、ASK1（6q23）、TAB2（6q25）擴增。
- Myxoid/round cell：特異性 t(12;16)(q13;p11) 融合 DDIT3 與 FUS；一小亞群為 t(12;22)(q13;q12)，由 EWSR1 取代 FUS。此等轉位幾乎見於所有病例。
- Pleomorphic：無一致細胞遺傳異常，常見複雜異常伴 TP53 突變。
組織病理特徵
- Dedifferentiated：定義為 well-differentiated liposarcoma 突然轉變至較高級非脂肪生成肉瘤。部分病例難以辨識 well-differentiated 區。可見灶狀 lipoblastic 分化。MDM2 與／或 CDK4 瀰漫陽性為恆定發現。
- Myxoid：由相當一致的星狀或紡錘細胞（含小空泡）構成，置於由 acid mucopolysaccharide 組成的黏液樣基質中。Mucin pooling 造成 lymphangioma-like 型態常見。強調有複雜叢狀的薄壁微血管網（chicken-wire 或「crow’s feet」型態）。有絲分裂稀少，lipoblasts 最易於腫瘤周邊辨識。較具細胞密度、由均勻卵圓至圓形較大細胞（核深染、胞質不顯）構成者表示 round cell 變化，與較具侵襲性行為相關，稱 combined myxoid and round cell liposarcoma 或 high-grade myxoid liposarcoma。
- Pleomorphic：由高度多形性紡錘細胞、lipoblasts 與眾多多核多空泡巨細胞構成。辨識 lipoblasts 對與其他 pleomorphic sarcomas 區分至關重要。
- Lipoblasts 於各病灶中存在量不一，於 myxoid 與 well-differentiated liposarcoma 可能缺如或極難尋。pleomorphic liposarcoma 中 lipoblasts 典型大小高度不一、含一個以上邊界清楚的脂質空泡、核不規則深染（有時多個），核緣被脂滴呈扇貝狀壓痕。
免疫組化與鑑別診斷
- dedifferentiated liposarcoma：MDM2 與／或 CDK4 核陽性多數可助確診。
- myxoid liposarcoma：以其獨特血管型態與其他肉瘤區分；困難病例可能需分子確認。Myxofibrosarcoma 以缺乏 lipoblasts 與較多變多形性辨識。與 lipoblastoma 區分可能極難或不可能，但 liposarcoma 於兒童極罕見。
- pleomorphic liposarcoma：基於在多形性肉瘤背景中辨識 lipoblasts，且缺乏 MDM2 與／或 CDK4 核陽性。
- 補充：myxoid 之 round cell 成分 S100 於 lipoblasts 陽性、於 round cell 成分不定陽性；cancer testis antigen NY-ESO-1 於此組腫瘤一致陽性。
