臨床特徵 (Clinical Features)
- 心臟外橫紋肌瘤 (extracardiac rhabdomyomas) 非常罕見，通常為深部 (deep-seated) 病灶，因此很少向皮膚科醫師求診。
致病機轉與組織學特徵 (Pathogenesis and Histologic Features)
生殖器型 (Genital type)
- 典型可見梭形 (spindle) 或帶狀 (strap-shaped) 細胞的增生，這些細胞具有豐富的嗜伊紅性 (eosinophilic) 細胞質，內含橫紋 (cross striations)，並嵌埋於纖維性間質 (fibrous stroma) 中。
- 不會出現多形性 (pleomorphism) 或有絲分裂活性 (mitotic activity)。
- 已辨識出一種以緻密膠原性間質 (dense collagenous stroma) 為特徵的硬化性變異型 (sclerosing variant)，主要見於男性的睪丸旁 (paratesticular) 位置，極少見於女性。
成人型 (Adult type)
- 曾有一例報導帶有 t(15;17)(q24;p13)。
- 成人型幾乎完全由大型的圓形、多角形 (polygonal) 或帶狀細胞 (strap-shaped cells) 組成，這些細胞具有豐富的嗜伊紅性細胞質（橫紋肌母細胞 [rhabdomyoblasts]）（Fig. 35.428）。橫紋 (cross-striations) 易於辨識，且常出現桿狀內含物 (rod-like inclusions)（Fig. 35.429）。嗜酸性變化 (oncocytic change) 極為例外罕見。
鑑別診斷 (Differential Diagnosis)
- 上述任一病灶與橫紋肌肉瘤 (rhabdomyosarcoma) 的鑑別，可藉由其缺乏有絲分裂活性、核多形性 (nuclear pleomorphism) 或浸潤性生長型態 (infiltrative growth pattern) 來達成。
- 免疫組織化學 (immunohistochemistry) 顯示對 muscle-specific actin、myoglobin 與 desmin 呈陽性。亦可見對 S100 protein 與 SMA 呈非常局灶性 (very focal) 的陽性。
胎兒型 (Fetal type)
- 已記載 PTCH1 基因座 (locus) 的異常，提示活化的 Hedgehog signaling 在胎兒型橫紋肌瘤 (fetal rhabdomyoma) 的致病機轉中扮演角色。
- 胎兒型幾乎完全由黏液樣間質 (myxoid stroma) 中未成熟的圓形至梭形橫紋肌母細胞 (rhabdomyoblasts) 組成，並向周邊呈現朝向更嗜伊紅性細胞的漸進性成熟 (progressive maturation)（這些細胞常顯示橫紋）（Figs 35.430 與 35.431）。
- 免疫組織化學顯示腫瘤細胞對 muscle-specific actin、myoglobin 與 desmin 呈陽性。對 SMA、GFAP 與 S100 protein 可能呈現局灶性 (focal) 陽性。
