黑色素細胞痣 (melanocytic nevus)
可見噬黑素細胞 (melanophages)，有時並伴有輕度的血管周圍慢性發炎細胞浸潤。
在偶見的病灶中，網狀真皮 (reticular dermis) 含有大量錯構瘤性 (hamartomatous)、不規則、增大的平滑肌纖維 (smooth muscle fibers)（圖 25.21）。25–28 文獻亦曾記載與之相關的真皮纖維化 (dermal fibrosis)、皮脂腺增生 (sebaceous hyperplasia)、皮脂腺痣 (nevus sebaceous)、局部痤瘡樣病灶 (localized acneiform lesions)、孤立性叢狀神經纖維瘤 (solitary plexiform neurofibroma)、第一型神經纖維瘤病 (neurofibromatosis type I)、基底細胞癌 (basal cell carcinoma)、複合型黑色素細胞痣 (compound melanocytic nevus)、色素性上皮樣黑色素細胞瘤 (pigmented epithelioid melanocytoma)、貧血性痣 (nevus anemicus)、扁平苔癬 (lichen planus)、汗斑 (pityriasis versicolor)、提示發炎性線狀疣狀表皮痣 (inflammatory linear verrucous epidermal nevus) 之乾癬樣皮膚炎 (psoriasiform dermatitis)，以及伴有顆粒細胞層局部減少之過度角化 (hyperkeratosis with focal diminution of granular cell layer)（如魚鱗病樣變化，ichthyotic changes）。3,28–38
melanocytic nevus（普通痣，banal nevus）是一種良性腫瘤，通常在兒童期與青春期出現。其性別分布相等。一般白人個體一生中可預期會發展出 15–40 個此類病灶，在第三個十年 (third decade) 達到最大數目，之後逐漸消退，至第八及第九個十年時幾近完全消失。1–5
除了源自有毛皮膚 (hair-bearing skin) 者之外，melanocytic nevi 亦可發生於無毛皮膚 (glabrous skin)、指甲與趾甲下、結膜 (conjunctivae) 與葡萄膜束 (uveal tract)，以及黏膜 (mucosae)（圖 25.22）。它們具有規律且組織學上有明確定義的自然病程，此病程在某種程度上可由其臨床外觀加以預測。其起源是上皮 (epithelium) 下部出現一個黑色素細胞增生 (melanocytic proliferation) 的病灶（接合部活性，junctional activity），即所謂的接合部痣 (junctional nevus)。此進展為黑色素細胞同時存在於表皮與真皮中，即為複合痣 (compound nevus)。進一步發展則形成完全位於真皮內的病灶，稱為真皮痣 (dermal nevus)。6
超微結構研究顯示基底層角質形成細胞 (basal keratinocytes) 內複合型黑素小體 (compound melanosomes) 的數目與大小增加。3 每個複合體 (complex) 中黑素小體 (melanosomes) 數目的增加亦可能明顯可見。
