Eleven cases of malignant subungual onycholemmal tumor have been described, characterized by abrupt (without a granular cell layer) onycholemmal keratinization.16,33โ€“35 Based on the relative number of onycholemmal cysts and solid lobules of atypical cells, they have been classified as malignant onycholemmal proliferating cyst33 or onycholemmal carcinoma, respectively.16,34 The tumors are observed in elderly people and have a long evolution time, with an infiltrative growth pattern. Small squamous pearls and cells exhibiting individual keratinization are lacking35. Genuine bone invasion is rare. These tumors may represent an unusual variant of SCC originating from the nail bed epithelium, more precisely the nail isthmus.16,35

In subungual metastasis, the most frequent sites of primary tumor are the lung and the genitourinary tract.19,20
