臨床特徵 (Clinical Features)

痛性脂肪症 (adiposis dolorosa)（即 Dercum disease）是一種罕見疾病，其特徵為呈斑塊樣 (plaque-like) 分布、界限清楚且脂肪增多的疼痛區域。通常多處身體部位受累，但好發於臀部、下肢與腹部，尤其是近關節 (juxta-articular) 區域。

侷限型 (localized) 疾病非常罕見，乳房亦可能受累。本病在女性遠較常見，多發生於人生第四至第六個十年，尤其是停經後，且患者常合併肥胖與心理問題。偶有病例以體染色體顯性 (autosomal dominant) 方式遺傳。已有文獻記載一例與使用 corticosteroids 相關的病例、另一例合併高膽固醇血症 (hypercholesterolemia) 與嚴重動脈粥樣硬化 (atherosclerosis) 的患者，以及一例合併心房間隔 (interatrial septum) 脂肪瘤樣肥厚 (lipomatous hypertrophy) 的病例。疼痛主要因觸診而產生，但也可能自發性發生，或因活動而引起。

已有人提出本病的新分類：(1) 不伴脂肪瘤的全身瀰漫型 (generalized diffuse without lipomas)；(2) 伴脂肪瘤的全身瀰漫型 (generalized diffuse associated with lipomas)；(3) 侷限結節型 (localized nodular)；以及 (4) 近關節型 (juxta-articular)。
