疾病定義與分類
- 毛根鞘囊腫 (trichilemmal cyst，又稱 pilar cyst) 的囊壁重現 (recapitulate) 了毛囊在 follicular isthmus 層級的 outer root sheath。有人建議應將這類囊腫重新命名為 isthmic-catagen cysts。
- 其起源尚不清楚，但有人認為它們是由 external root sheath 出芽 (budding off) 而成，屬於一種由遺傳決定的結構性偏差 (genetically determined structural aberration)。
- 家族性發生見於 75% 的患者，其型態提示為 autosomal dominant inheritance（體染色體顯性遺傳）。
臨床特徵 (Clinical Features)
- 毛根鞘囊腫在 90% 的病例位於頭皮 (scalp)；其中 30% 為單發 (solitary)，70% 為多發 (multiple)（Fig. 34.18）。
- 也可見於不尋常的部位，例如指（趾）腹 (the pulp of …)。
